Job Description Summary
The Clinical Pharmacist is a licensed professional who is responsible for providing safe and appropriate pharmacy services to Option Care patients in accordance with the policies and procedures of the Option Care pharmacy practice professional standards and applicable regulatory entities. This entry‑level position introduces the practice of clinical pharmacy in an alternate setting. The staff Clinical Pharmacist coordinates patient services with the Nursing Department, monitors patient progress toward goals, and educates patients, caregivers, staff and other professionals regarding appropriate medication use.
Job Responsibilities (listed in order of importance and/or time spent)- Determine the suitability of individual patients for home care, collecting and organizing patient‑specific information and assessing suitability based on Option Care policies, medical condition, and prescribed medication therapy.
- Design, recommend, monitor and evaluate patient‑specific pharmacy care plans for home care patients, making recommendations for modifications to medication therapy or monitoring parameters as appropriate.
- Provide medication‑use education to Option Care staff, other professionals, home care patients and caregivers using effective patient education techniques.
- Ensure continuity of pharmaceutical care to and from the home and other patient‑care settings, following systematic procedures to communicate pertinent patient information. Prepare and dispense medications according to policies, calibrate equipment, prepare labels, inspect final medication, and comply with state, federal, USP, and DEA regulations.
- Manage the use, maintenance, and troubleshooting of medication administration equipment and related equipment, participate in incident reporting, and complete QAR documentation.
- Provide concise, timely responses to drug information requests from health care providers and home care patients, formulate responses based on literature analysis, attend educational seminars, and meet state‑mandated CEU requirements.
- Manage the use of investigational drug products according to established regulations and Option Care policies, utilize pharmacy support personnel, prioritize workload, and ensure accuracy of support staff work.
- Comply with accreditation, legal, regulatory, and safety requirements, and maintain confidentiality of patient and proprietary information in accordance with legal and ethical guidelines.
Does not have supervisory responsibilities.
BasicEducation and/or Experience Requirements
- Graduate of an accredited School of Pharmacy.
- Current, active Pharmacy license granted by the State Board of Pharmacy.
- At least one year of experience in sterile IV admixture procedures, or knowledge of non‑sterile compounding, immunizations, and medication therapy management, completion of an ASHP accredited Pharmacy Residency Program, an Advanced Pharmacy Practice Experience associated with an accredited school of pharmacy, or equivalent experience approved by the VP of Pharmacy or Senior VP of Clinical Services.
- Experience providing customer service to internal and external customers, meeting quality standards for services and evaluating customer satisfaction.
- Basic proficiency in Microsoft Word, Excel, and PowerPoint sufficient for routine document preparation and reporting.
Willing to travel up to 5 % of the time for business purposes within the state.
Preferred Qualifications & Interests (PQIs)- Completion of an ASHP accredited Pharmacy Residency Program or Advanced Pharmacy Practice Experience associated with an accredited school of pharmacy.
- Experience in home care, hospital, or other clean‑room setting.
At least 1 year of experience in indirect management of team members, including assistance in the development, training, and assignment of work to other team members. May perform other duties as assigned.
