About the Role:
The Memory Care Coordinator is responsible for identifying and meeting the needs of residents with Dementia, Alzheimer's, and other memory loss conditions. As a key member of our Nursing team, you will create an environment that minimizes risks and maximizes the safety and well-being of our residents, assisting them in managing various stages of memory loss.
Essential Duties/Responsibilities:
Collaborate with the Activities Director to create enrichment programs tailored for residents in all stages of Dementia and Alzheimer's that help preserve their skills.
Help promote program activities to residents and family members to get them excited about participating.
Track and document daily activities.
Serve as emotional support for residents and visiting family members.
Establish and organize a monthly activity calendar and ensure needed supplies are available.
Cooperate with the Director of Nursing and other facility staff members to ensure the health and social services available to residents are top-quality and their needs are adequately being met.
Provide daily oversight of the day-to-day operations of the Memory Care Services in partnership with the Clinical Leadership team.
Plan, develop, coordinate, execute, and continuously evaluate the Memory Care program needs.
Consult with members of management to report resident changes in condition and recommend adjustments in the level of care and service based on the needs of the resident.
Manage assessments to determine the level of care and staffing needs.
Ensure that all State and Federal guidelines are met as per program specifications.
Maintain current knowledge of Alzheimer's/Dementia topics.
Adhere to budget guidelines for the department.
Maintains the confidentiality of all resident care information and complies with all HIPAA standards.
Ensures that all residents are treated fairly, with kindness, dignity, and respect.
Perform other duties as assigned.
Qualifications:
Associate's degree in social work, Recreation Therapy, Psychology, or a related field.
Entry-level experience: one or more years combined experience in Alzheimer's/Dementia Care, Senior Recreation Programming, and management is required.
Special
Certifications:
Activity Director Certified, Certified Dementia Practitioner, or Therapeutic Recreation Assistant a plus.Must be able to communicate in the capacity necessary to perform the tasks required.
Must have patience, tact, a cheerful disposition, and enthusiasm, as well as the willingness to handle difficult situations.
Must create and maintain an atmosphere of warmth, care, and support for residents.
