Job Summary
Graham Healthcare Group is seeking a Registered Nurse (RN) Home Health for a nursing job in Itasca, Illinois. This full‑time position supports patients in Schaumburg, Arlington Heights, Rolling Meadows and surrounding areas. The role includes a $10,000 first‑year success bonus for key achievements during the first year.
Responsibilities- Lead and collaborate as a case manager to manage and deliver patient care plans in a team‑based environment.
- Provide home health care to patients, performing visits approximately 8 hours per shift, 40 hours per week.
- Maintain timely documentation of patient care, charting and critical deadlines.
- Self‑manage time and schedule, ensuring efficient use of travel and workload.
- Utilize tablets, mobile phones and EMR software to support care delivery.
- RN license in the state of work.
- Graduate from an approved school of practical nursing.
- One year of RN experience with clinical competence in patient care.
- Current driver’s license and ability to drive ~20% of the day to patient locations.
- Demonstrated skills in managing time, schedule, and EMR technology.
- Commitment to consistently meet critical charting deadlines.
