As a Licensed Practical Nurse (LPN) at Cayuga Ridge Nursing & Rehabilitation Center, you will have the opportunity to make a meaningful difference in the lives of our residents. Our 144-bed Skilled Nursing & Rehabilitation Center is located on the beautiful West bank of Cayuga Lake in Ithaca, NY, and we are proud to offer a comprehensive range of services to meet the unique needs of each individual.

About the Role:

• Provide high-quality nursing care to our residents, focusing on their physical, spiritual, and emotional well-being

• Work collaboratively with our interdisciplinary team of healthcare professionals to develop and implement individualized care plans

• Administer medications, perform wound care, and manage chronic conditions in accordance with established protocols

• Communicate effectively with residents, families, and healthcare providers to ensure seamless transitions and care coordination

Responsibilities:

• Provide direct patient care

• Assist with activities of daily living (ADLs)

• Report changes in resident condition to the nursing team

• Maintain accurate and up-to-date records of patient care and treatment

• Participate in quality improvement initiatives to enhance patient outcomes and satisfaction
