Help Families Move from Crisis to Confidence

Life Ways is seeking a compassionate and motivated ICCW Coordinator to facilitate the Wraparound process using the Michigan Department of Health and Human Services (MDHHS) fidelity model. In this role, you'll partner with children and families to build strength-based, individualized plans that promote recovery, resilience, and long-term success.

• Facilitate Child and Family Team meetings and develop Family Plans and Individual Plans of Service.
• Coordinate and monitor services and supports, ensuring continuity of care.
• Support youth eligible under the Severe Emotional Disturbance (SED) waiver and other qualifying populations.
• Advocate for families by collaborating with providers, schools, and community partners.
• Assist with crisis intervention, discharge planning, and life transitions.
• Provide services in home, school, and community settings, including some non-traditional hours.
• Maintain accurate documentation and compliance with model, agency, and contractual requirements.
