Job Overview

Graham Healthcare Group is seeking a Home Health Physical Therapist for a position in Jackson, Michigan. This role supports patients in Jackson, MI and surrounding areas on an every-other-weekend schedule, approximately 16 hours per week. The position is part-time (8‑hour shifts) and staff‑based at Residential Home Health and Hospice.

• Provide home health physical therapy services to patients, collaborating with the care team to manage and deliver patient care plans.
• Maintain accurate charting and consistently meet critical deadlines for documentation.
• Adhere to a flexible schedule that includes substantial travel (~20% of the day) to patient locations.
• Utilize tablets, smart phones, and EMR software effectively.
• Self‑manage time and schedule to ensure optimal productivity on an every-other-weekend basis.

• Bachelor’s or Master’s degree in Physical Therapy from an accredited college (required).
• Licensed Physical Therapist issued by the state in which you will be working (required).
• Minimum of one year of healthcare experience in a relevant setting.
• Valid driver’s license and capability to drive to patient locations.
• Proficiency with tablets, mobile phones, and EMR software.
• Strong self‑management skills for time and schedule control.

Compensation includes pay for patient care visits, non‑visit activities, paid days off, shift premium pay, and a first‑year success bonus. Compensation potential varies by market; the low end of the range reflects an average annual compensation based on a fully productive caseload, while the high end reflects those who exceed productivity.
