Social Services Designee

Overview

Join our EPIC team at Cascade Senior Care Center! The Social Worker provides social services to residents and their families to assist them in dealing with the impact of illness and extended care placement. This position participates in resident care management and ensures that needs are met through Social Service programming and appropriate referrals to outside agencies and community resources.

What you will earn: up to $70,000 salary per year based on years of experience;
Yearly Wage Increases.

• Provide social services to residents and families to support coping with illness and extended care placement.
• Participate in resident care management and ensure needs are met through Social Service programming.
• Make appropriate referrals to outside agencies and community resources.

• Degree in Social Work, or in related human services field (e.g., Sociology, Special Education, Rehabilitation Counseling, Psychology) is preferred but not required.
• Licensed in Social Work preferred but not required.
• 1–2 years of supervised social work experience in a health care setting working directly with individuals.
• Excellent customer service skills with a commitment to delivering excellent, passionate, innovative care.
