DescriptionUnder the direction of the Director of Behavioral Health, the Manager of Integrated Behavioral Health and Case Management Services assists in the oversight of integrated behavioral health programming, develops and oversees integrated behavioral health education and training programs, and oversees the clinical and administrative supervision of the behavioral health integration specialists (BHS). This role also provides direct supervision to Fenway's Community Health Workers (CHWs)/Care Navigators/Care Coordinators, as assigned, and leads the development of an organization-wide case management program in partnership with nursing, primary care, Behavioral Health, and Accountable Care Organizations (ACOs).
The Manager maintains a clinical practice, providing direct behavioral health counseling, advocacy, and referrals to community resource services, and carries a designated day of patient care hours each week.
- Maintains one day per week of dedicated patient care hours, structured according to departmental needs, which may include drop-in clinic hours, bridge appointments, or longer-term therapy visits.
- Depending on departmental needs, may carry a small panel of behavioral health clients.
- Follows the SBIRT model to provide psychosocial assessments and mental health and substance use evaluations for medical patients.
- Provides direct behavioral health treatment with an emphasis on short-term, evidence-based psychotherapeutic care within the primary care setting.
- Assists the Director of Behavioral Health with behavioral health integration program strategy, design, and development.
- Maintains awareness of industry best practices for deep integration of behavioral health expertise within primary care and medical team-based settings to support practice innovation initiatives.
- Ensures that newly hired behavioral health specialists receive a comprehensive orientation.
- In collaboration with nursing and primary care leadership, assists in the design of the daily operations of the full clinical care team, inclusive of care coordination, healthcare navigation, and health education informed by patients' social drivers of health.
- Makes employment-related decisions regarding assigned staff within the scope of defined authority and following established agency and program guidelines.
- Supports and continually improves the provision of professional, comprehensive, and effective clinical services for all BH clients.
- Provides direct clinical and administrative supervision to Community Health Workers (CHWs)/Care Navigators/Care Coordinators, as assigned.
- Oversees daily operations, caseload management, and professional development of the CHW/Care Navigator/Care Coordinator team.
- Establishes and monitors performance expectations, workflows, and quality standards for CHW/Care Navigator/Care Coordinator services.
- Supports continuity and stability of the CHW/Care Navigator/Care Coordinator team, including onboarding, training, and coordination with clinical and medical partners.
- Partners with the Ryan White Project Director to ensure accurate and timely completion of Ryan White reporting requirements as assigned.
- Oversees CHW/Care Navigator/Care Coordinator responsibilities related to HIV-specific case management supported by Ryan White funding.
- Ensures CHW/Care Navigator/Care Coordinator documentation and service delivery align with Ryan White grant requirements and reporting timelines.
- Leads the design and implementation of an organization-wide case management program and plan to address the social drivers of health impacting Fenway's patient population.
- Collaborates with nursing, primary care, Behavioral Health, and ACOs to align case management services across departments.
