In this role you will be responsible for:

• Completing initial evaluation and re‑evaluating the patient's nursing needs during each visit.
• Assuming primary responsibility for caseload, including assessing, planning, coordinating, implementing, and evaluating the patient plan of care.
• Completing assessments specific to the provision of hospice care, such as identifying home safety risks, suitability of home environment, availability and capacity of support systems and community resources.
• Providing appropriate support at the time of death and performing bereavement assessment if requested.
• Providing positive, supportive communication to physicians, patients, families, facilities, visitors, and other organization personnel.

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
• Current State License as a Registered Nurse
• Minimum 1 year of experience as a nurse in a clinical care setting
• Demonstrated nursing skills and ability to handle multiple priorities, documentation requirements, and deadlines.
• Ability to assess and respond to the needs of patients, families, and caregivers in varied settings, including home or nursing facility environments.
• Composure in dealing with ill, disabled, terminal patients and family emotional stress, and tolerance of individual lifestyles.
• Reliable transportation and valid automobile insurance coverage.

• BSN preferred.
• Hospice or home health experience preferred.

$80,000 – $85,000 USD
