In this role you will be responsible for:
- Completing initial evaluation and re‑evaluating the patient's nursing needs during each visit.
- Assuming primary responsibility for caseload, including assessing, planning, coordinating, implementing, and evaluating the patient plan of care.
- Completing assessments specific to the provision of hospice care, such as identifying home safety risks, suitability of home environment, availability and capacity of support systems and community resources.
- Providing appropriate support at the time of death and performing bereavement assessment if requested.
- Providing positive, supportive communication to physicians, patients, families, facilities, visitors, and other organization personnel.
- Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
- Current State License as a Registered Nurse
- Minimum 1 year of experience as a nurse in a clinical care setting
- Demonstrated nursing skills and ability to handle multiple priorities, documentation requirements, and deadlines.
- Ability to assess and respond to the needs of patients, families, and caregivers in varied settings, including home or nursing facility environments.
- Composure in dealing with ill, disabled, terminal patients and family emotional stress, and tolerance of individual lifestyles.
- Reliable transportation and valid automobile insurance coverage.
- BSN preferred.
- Hospice or home health experience preferred.

- 7:00 am – 7:00 pm, Friday through Sunday
- Every weekend schedule
$80,000 – $85,000 USD
