About the Role

You will practice nursing with autonomy and meaningful impact as a Home Health Registered Nurse. Responsibilities include comprehensive skilled nursing care in patients’ homes, managing chronic conditions, and aiding recovery, with full caseload ownership from admission to discharge. You will collaborate with physicians and a multidisciplinary team while exercising clinical judgment that shapes patient outcomes.

What You Will Do
  • Conduct initial evaluations and ongoing OASIS assessments at appropriate time points.
  • Deliver skilled nursing services per physician orders.
  • Initiate diagnostic, preventive, and rehabilitative nursing procedures.
  • Perform phlebotomy, medication administration, and wound care.
  • Monitor and respond promptly to changes in patient condition.
  • Manage your caseload: assess, plan, coordinate, implement, and evaluate care.
  • Develop individualized care plans and modify them as needed.
  • Coordinate patient discharge and maintain accurate documentation within 24 hours of visits.
  • Teach and empower patients and families with home nursing procedures and self‑care techniques.
  • Collaborate with physicians, therapists, social workers, and other professionals.
  • Supervise home health aides, LPNs/LVNs, and paraprofessionals.
  • Communicate findings, report unsafe conditions, and participate in quality improvement initiatives.
Required Qualifications
  • Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
  • Current State License as a Registered Nurse.
  • Minimum 1 year experience as a licensed RN in a clinical care setting.
  • Reliable transportation with valid automobile insurance.
  • Willingness to travel daily for home visits within the service area.
Preferred Qualifications
  • Bachelor of Science in Nursing (BSN).
  • Experience in a home health environment.
  • OASIS certification or willingness to obtain.

  • Knowledge of Medicare, Medicaid, and third‑party payer requirements.

Physical demands include standing, walking, bending, and lifting up to 50 lbs with assistance.

    8:00 am–5:00 pm, Monday‑Friday (occasional on‑call weekend rotation)

Pay Range: $75,000 – $85,000 USD
