Registered Nurse – Behavioral Home Health

Full‑time position involving behavioral home health and skilled home health visits across assigned territory, 4–6 visits per day.

• Assess assigned caseload of behavioral health patients and families to identify physical, psychosocial, and environmental needs based on documentation, clinical records, interdisciplinary team reports, after‑hours reports, and on‑site evaluations.
• Assume primary responsibility for the caseload, including assessing, planning, coordinating, implementing, and evaluating the plan of treatment.
• Obtain data on physical, psychological, social, and spiritual factors that may influence patient/family/caregiver health status and incorporate that data into the plan of care.
• Make the initial evaluation and re‑evaluate nursing needs during each visit.
• Make follow‑up visits according to patient, family, and facility needs.
• Communicate significant findings, problems, and changes in health condition, environment, or unsafe facility conditions to the clinical supervisor, physician, facility, and other personnel involved with patient care.
• Revise the plan of care in response to identified patient care issues and notify the clinical supervisor, physician, and other team members.
• Counsel, instruct, and include the patient, facility, and family in following the interdisciplinary plan of care.
• Teach behavioral health philosophy and pain and symptom management to staff in alternate care settings such as nursing homes, hospitals, and assisted‑living facilities.
• Initiate, supervise, and document observance of the behavioral health aide and LPN delivery of care per agency policy.
• Inform supervisor and/or facility supervisor of unusual or potentially problematic nursing facility issues.
• Provide appropriate support at the time of death and perform bereavement assessment.

• Current, unrestricted state RN license.
• At least 1 year of experience in a clinical care setting as a nurse.
• Behavioral health/psychiatric nursing experience of at least 6 months to 1 year in an inpatient setting.
• Skilled Home Health experience strongly preferred.
• Ability to travel within branch/office coverage area; dependable vehicle, valid driver’s license, and current auto insurance.
• Physical ability to lift, push, pull, and support up to 50 pounds.
• Associate’s or Bachelor’s Degree in Nursing preferred.
• Previous Home Health experience strongly preferred.
• Previous experience with Home Care Home Base preferred.
