Registered Nurse RN Home Health

Overview

At Elara Caring, we provide high‑quality home health care to more than 60,000 patients each day in their preferred home setting. The Registered Nurse (RN) Home Health will play a critical role in delivering compassionate care across our coverage areas, including Chelsea, Manchester and Grass Lake, MI.

• Ensure all activities align with Elara Caring’s mission and board directives.
• Maintain patient and staff privacy and confidentiality in accordance with HIPAA.
• Assess the assigned caseload of patients and families to identify physical, psychosocial and environmental needs.
• Assume primary responsibility for the caseload: assess, plan, coordinate, implement, and evaluate the plan of care.
• Gather physical, psychological, social and spiritual data that may influence patient/family/caregiver health status and incorporate it into the care plan.
• Conduct initial and routine evaluations, and re‑evaluate nursing needs during each visit.
• Perform follow‑up visits per patient, family and facility needs.
• Communicate significant findings, problems and changes in health condition, environment or unsafe facility conditions to Clinical Supervisor, physician, facility and other team members.
• Revise the plan of care in response to identified patient care issues and notify the Clinical Supervisor, physician, facility and other team members.
• Report to the Clinical Manager or Clinical Supervisor regarding patient status and care progress.

• Current, unrestricted state RN licensee.
• Minimum of 1 year of clinical nursing experience.
• Ability and willingness to travel within the branch coverage area (up to 50% travel).
• Physical capacity to lift, push, pull and support up to 50 pounds, including patient positioning and equipment movement.
• Dependable vehicle, valid driver’s license and current auto insurance as required by state law.
• Associate or Bachelor’s degree in Nursing preferred.
• Previous Home Health or Hospice experience preferred.
• Experience with Home Care Home Base preferred.
