Overview

The Registered Nurse (RN) Home Health will play a critical role in delivering compassionate care across our coverage areas, including Chelsea, Manchester and Grass Lake, MI.
Responsibilities- Ensure all activities align with Elara Caring’s mission and board directives.
- Maintain patient and staff privacy and confidentiality in accordance with HIPAA.
- Assess the assigned caseload of patients and families to identify physical, psychosocial and environmental needs.
- Assume primary responsibility for the caseload: assess, plan, coordinate, implement, and evaluate the plan of care.
- Gather physical, psychological, social and spiritual data that may influence patient/family/caregiver health status and incorporate it into the care plan.
- Conduct initial and routine evaluations, and re‑evaluate nursing needs during each visit.
- Perform follow‑up visits per patient, family and facility needs.
- Communicate significant findings, problems and changes in health condition, environment or unsafe facility conditions to Clinical Supervisor, physician, facility and other team members.
- Revise the plan of care in response to identified patient care issues and notify the Clinical Supervisor, physician, facility and other team members.
- Report to the Clinical Manager or Clinical Supervisor regarding patient status and care progress.
- Current, unrestricted state RN licensee.
- Minimum of 1 year of clinical nursing experience.
- Ability and willingness to travel within the branch coverage area (up to 50% travel).
- Physical capacity to lift, push, pull and support up to 50 pounds, including patient positioning and equipment movement.
- Dependable vehicle, valid driver’s license and current auto insurance as required by state law.
- Associate or Bachelor’s degree in Nursing preferred.
- Previous Home Health or Hospice experience preferred.
- Experience with Home Care Home Base preferred.
