Nurse Practitioner, Geriatric Nurse Practitioner
Practice medicine in a way that prioritizes time, connection, and meaningful patient relationships. As a Nurse Practitioner or Physician Assistant in Physician Services, you will provide primary and consultative care to geriatric and medically complex patients, often in their homes or care settings, with the autonomy to focus on patient‑centered care that improves comfort, function, and quality of life.
In this role you will conduct comprehensive medical evaluations, develop and manage individualized treatment plans, prescribe and monitor medications, and coordinate closely with physicians, caregivers, and interdisciplinary team members. You will serve as a key clinical decision‑maker and help patients and families better understand diagnoses, manage chronic illness, and avoid unnecessary hospitalizations.

Greater Jackson and surrounding areas - This role is a high‑travel position and requires daily travel within the service area to provide care to patients in their homes, assisted living facilities, or nursing homes.
- Monday to Friday, 8 a.m. to 5 p.m.
- Master’s or doctoral degree as a Board‑Certified Nurse Practitioner or Physician Assistant
- Current State License as a Nurse Practitioner or Physician Assistant
- Reliable transportation, valid driver’s license, and valid automobile insurance coverage
- Minimum 2 years of clinical experience in a post‑acute setting as a Nurse Practitioner or Physician Assistant
- Adult, Family, or Gerontology certification
- Experience with geriatric patients
- Adherence to nursing standards of practice
- Ability to assess and respond to the needs of patients, families, and caregivers in post‑acute care settings
- Skill in nursing practice, coping with family emotional stress, and tolerance of diverse lifestyles
- Ability to handle multiple priorities, documentation requirements, and deadlines
$110,000—$125,000 USD (base annual salary with monthly bonus potential)
