Travel Social Worker – Care Coordinator (Discharge Planning)

Care Setting: Acute Care

Position Details:
Social Worker needed for an acute care, large academic medical center and Level I Trauma facility in Jackson, MS. This role focuses on inpatient discharge planning and social work case management across all inpatient units. The facility manages complex patient populations, including medical and social discharges.

The Social Work team includes approximately 15 full-time and 17 part-time social workers. Travelers will work alongside the care management team and will not be working independently.

Responsibilities:

• Inpatient discharge planning and social work case management

• Manage caseload of approximately 15–20 patients/consults per day

• Coordinate complex placements and transitions of care

• Collaborate with interdisciplinary teams including RT and Pharmacy as needed

• Utilize EPIC EMR for documentation and care coordination

• Assess discharge readiness and address social barriers to discharge

Requirements:

• Minimum 2 years of acute care social work case management experience

• Previous experience in a large academic medical center and/or Level I Trauma Center required

• Active Mississippi Social Work license (LMSW or LCSW) required at time of submission

• BLS certification

• EPIC charting experience required

• Strong understanding of discharge planning and readiness
