RN-Transplant Coordinator- Bone Marrow Transplant Unit

Job Summary

Transplant coordinator responsible for the coordination of solid organ transplantation (liver, kidney, pancreas and multi‑organ) for all pediatric and adult recipients. The role serves as a key multidisciplinary team member, overseeing patient care from pre‑transplant to post‑transplant, educating patients and families, and ensuring regulatory compliance.

• Licensed registered nurse (RN) for at least one (1) year.

• Valid RN license.
• Basic Life Support (BLS).

• Experience in an intensive care or transplant setting.
• BSN degree.
• Certified Clinical Transplant Coordinator (CCTC).
• Advanced Cardiac Life Support (ACLS).

• Critical thinking, time management, and organizational skills.
• Strong presentation and decision‑making abilities.
• Understanding of healthcare industry trends, medical procedures, patient assessment, and regulatory requirements.
• Excellent written and oral communication; ability to present education materials to diverse audiences.
• Attention to detail and effective documentation with physicians, organ procurement organizations, operating room, intensive care unit, and emergency department staff.

• Provide daily clinical assessment of transplant patients throughout all care phases.
• Coordinate evaluation process for pre‑transplant patients to ensure optimal use of hospital resources.
• Assess post‑transplant organ function and general health status; determine need for additional assessment or intervention.
• Schedule diagnostic studies or procedures with fiscal responsibility.
• Participate in protocol development and standards of care for transplant patients.
• Assist nurses and staff in planning, implementing, and evaluating patient care.
• Monitor routine care outcomes to determine quality and performance improvement processes.
• Provide on‑call coverage; serve as medical triage for outpatients in collaboration with providers on‑call.
• Coordinate transplant processes when a donor is available (24/7), communicating with physicians, organ procurement organizations, patients, families, and supporting teams.
• Review medical care, lab and procedural results; maintain medication reconciliation and triage care with transplant providers.
• Complete documentation as required by CMS and UNOS regulations.
• Communicate timely with multidisciplinary team and patients/families regarding all care issues.
• Coordinate with transplant administrators, financial coordinators, and case managers to plan nursing care with fiscal responsibility.
• Educate potential recipient and donor candidates, families, and caregivers throughout the continuum of care.
• Develop and disseminate patient education materials; identify external resources for patient education.
• Educate nurses and multidisciplinary team members on patient care issues and promote positive outcomes.
• Provide community education and consultation on transplantation and organ donation.
• Serve as a resource or preceptor in organ transplant for graduate and undergraduate nursing programs.
• Lead or participate in team conferences, rounds, or meetings regarding transplant patient care.
• Interpret research and published data for nursing staff concerning current transplantation issues.
• Incorporate research into evidence‑based practice and collaborate with medical team on appropriateness.
• Identify areas for study and participate in or design clinical research.
• Consult with unit leadership and resolve transplant‑related issues; advise referring physicians and nurses.
• Act as a community consultation resource regarding transplantation and organ donation.
• Model professional nursing through conduct, ethical decision‑making, critical thinking, and problem‑solving skills.
• Communicate effectively to maintain positive working relationships.
• Engage in strategic planning for university transplant and related departments.
• Participate in departmental committees, hospital‑wide committees, or task forces as needed.
• Engage in professional development and extend knowledge through presentations, publications, and other scholarly outlets.
