Registered Nurse - Hospital Case Management
Seeking an experienced Registered Nurse specializing in hospital case management to coordinate patient care, discharge planning, and utilization review for a Level 1 trauma hospital. The role requires quick adaptation to a fast-paced environment with a focus on acute inpatient medical case management.
Responsibilities:
- Coordinate and facilitate patient discharge planning across various inpatient units.
- Review and manage patient cases to ensure appropriate care and resource utilization.
- Collaborate with social workers, healthcare providers, and insurance companies to optimize patient outcomes.
- Conduct telephonic and in-person assessments for complex patient cases, ensuring compliance with hospital protocols.
- Maintain accurate documentation in EPIC and other clinical systems.
- Participate in unit orientation and team meetings to align patient care strategies.
Qualifications:
- Valid RN license with current registration.
- Minimum of 2 years of acute inpatient medical case management experience in a large, busy hospital setting.
- At least 1 year of recent experience (within the last 3 years) in a Level 1/academic medical center ED.
- Proven proficiency in EPIC charting and electronic medical records.
- Experience in discharge planning for patients with diverse medical diagnoses.
- Strong critical thinking and communication skills.
