About the Position:
* This is a time-limited position.
* Coordinates and facilitates statewide care coordination and follow-up services for children identified with a venous blood lead level at or above the current Centers for Disease Control and Prevention Reference Value. Maintains care coordination files for children with a venous blood lead level of 15 or higher. Works closely with providers to ensure children are receiving appropriate testing and follow-up procedures established by the Centers for Disease Control and Prevention.
Participate in state and local stakeholder meetings. Works closely with families and communities to raise awareness of lead poisoning. Assist with writing grants, policies and procedures, action plans and reports. Conducts telephone counseling to families referred from other health programs who are potentially at-risk for lead exposure. Assist with ordering supplies, processing invoices for payment, contract development and fiscal oversight.
Other duties as assigned.
What you'll need:
To be a successful candidate, you should be able to perform work of greater complexity, exercise greater independence in making decisions, and have working knowledge of social service policies, practices and techniques. Preferred Qualifications Typically requires a bachelor's degree and 3-5 years of experience. Appointing authority may require social worker's license.
