  • Align all activities with Elara Caring’s vision and the leadership of the Hospice team
  • Prevent unwanted hospitalizations by meeting patient and family needs in the home
  • Prepare and maintain accurate patient records, charts, and documents
  • Comply with applicable laws and regulations regarding Collaborative Agreements
  • Complete accurate billing and coding for patient encounters according to CMS guidelines
  • Initiate reimbursement for medically necessary visits
  • Provide training and continuing education for staff
  • Assist in developing clinical practice guidelines and standards supporting quality palliative care
  • Maintain confidentiality regarding patients, families, staff, and the organization
  • Collaborate and communicate with the Medical Director, attending physician, branch leadership, and IDG
  • Perform other assigned duties and projects
  • Travel up to 50% as needed to support business and patient needs
  • Report to the Branch Director
Requirements
  • Master’s Degree in Nursing
  • Certification through a national Nurse Practitioner Licensing board
  • 3+ years of clinical nursing experience
  • 1+ year of post-graduate clinical nursing experience in hospice and palliative care
  • Current licensure in the state of practice as a Nurse Practitioner
  • Maintain Nurse Practitioner licensure according to applicable laws and regulations
  • Active DEA license
  • Able and willing to travel up to 50% as needed
  • Prior experience in Family Medicine, Geriatrics, and psychiatry preferred
  • Board Certification in Palliative Care preferred
