- Provide Behavioral Home Health and Skilled Home Health visits across the assigned territory
- Complete 4–6 patient visits per day as a full-time employee
- Assess behavioral health patients and families to identify physical, psychosocial, and environmental needs
- Assume primary responsibility for the caseload, including assessing, planning, coordinating, implementing, and evaluating treatment plans
- Assess physical, psychological, social, and spiritual factors and incorporate findings into the plan of care
- Conduct initial evaluations and re-evaluate nursing needs during each visit
- Make follow-up visits according to patient, family, and facility needs
- Communicate significant findings, problems, health-condition changes, environmental concerns, and unsafe facility conditions
- Revise plans of care and notify the Clinical Supervisor, physician, facility, and team members
- Counsel and instruct patients, facilities, and families in following the Interdisciplinary Plan of Care
- Teach behavioral health philosophy and pain and symptom management to staff in alternate care settings
- Initiate, supervise, and document behavioral health aide and LPN care delivery
- Inform supervisors of unusual or potentially problematic nursing facility issues
- Provide support at time of death and perform bereavement assessments
- Report to the Clinical Manager or Clinical Supervisor
- Must have at least 6 months to 1 year of Behavioral/Psych nursing, preferably in inpatient
- Current, unrestricted state RN licensee
- 1+ years of experience in a clinical care setting as a nurse
- Behavioral health/psych experience as a nurse required - at least 6 months - 1 year in inpatient psych
- Skilled Home Health experience strongly preferred
- Able and willing to travel within branch/office coverage area
- Must have a dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws
- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
- Must be able and willing to travel 50%
- Associates Degree or Bachelors Degree in Nursing is preferred
- Previous Home Health Experience is strongly preferred
- Previous experience with Homecare Home Base is preferred
Demonstrates expertise in Behavioral Health Nursing, including assessment, planning, and implementation of treatment plans. Proficient in patient evaluations, interdisciplinary communication, and home health care delivery.
