Overview
Home Care Network (HCN) is seeking a highly skilled, experienced RN Case Manager to join our home health agency on a PRN basis and serve existing and new clients across our Southeast Ohio region, primarily serving Jackson, Vinton, and Gallia Counties.
More about the role:
- This is a PRN / As Needed schedule, with hours and shifts varying week to week, including some weeks with low or no hours.
- Candidates seeking long-term employment are preferred; however short-term or temporary employment will be considered.
- This position has the potential to grow into a consistent Part-Time (Hourly) or Full-Time (Salary) position if desired — not required.
- Typical travel distances include a 30-minute radius daily, with up to 60 minutes on occasion. Additional hours are available if willing to travel up to 2 hours as needed (optional, not required).
- Door-to-Door mileage reimbursement provided.
- At least 1 year of experience in case management is required. One or more years of experience in home health care, wound care, lab draws, and OASIS assessments is a plus.
- Services include starts of care (SOC), recertifications, resumptions of care (ROC), and other assessments and services, as well as new hire orientations and reviews.
- RN performs client assessments while directing, planning, and initiating appropriate actions in home care situations.
- RN manages/supervises a team of RN, LPN, Home Health Aide (HHA), and Personal Care Aides.
- RN communicates/document patient status/change in condition to the care team.
- RN promotes personal safety and a safe environment for clients and coworkers while performing job duties.
- Willing to travel to surrounding counties.
- Minimum of one (1) year of nursing supervisory experience; home care experience is preferred.
- Graduate of an accredited School of Nursing.
- Minimum of 1 year experience as a Registered Nurse, with an active RN license/ability to practice in the State of Ohio.
- CPR and First Aid Certifications required (or can be obtained prior to hire date through our agency or accredited third parties).
- 1 year of experience in case management required; home health experience highly preferred.
- Experience in supervisory role, wound care, lab draws, and OASIS assessments is a plus.
- Strong background in nursing, patient care, and confidentiality.
- Strong attendance and dependability history with previous employers.
- Desirable traits: reliable, honest, professional, compassionate, detail-oriented (in care and documentation), accountable, friendly, respectful, and positive.
- Familiarity with HIPAA regulations and medical terminology.
- Current Driver's License and reliable transportation.
- Part-time: $35-$40/hr
As an RN Case Manager, you will be instrumental in coordinating and providing patient care and ensuring optimal outcomes.
- Core skills in managed care, patient assessment, client care and treatment plan documentation, and discharge planning to oversee patient services and provide expert nursing care.
- Premium skills in critical care and infectious disease care will be valuable in handling complex cases; experience in home and community care and supervising will help lead and support a multidisciplinary team.
- Ensure effective communication between physicians, agency staff, service administrators, other caregivers and clinicians, and the client and family.

Regional Administrator, or Clinical Administrator/Director of Nursing

- Current CPR and First Aid Certifications; RN license active in Ohio.
- Familiarity with HIPAA regulations and medical terminology.
- Current Driver's License and reliable transportation.
