Heartland Hospice is seeking a Hospice RN / RN Case Manager to deliver personalized end-of-life care in patients' homes or other settings. You will lead nursing care based on each Plan of Care, perform assessments, and collaborate with physicians, CNAs, social workers, and chaplains to refine patient care plans.
Ideal candidates hold an active RN license, CPR certification, and at least 1 year of hospice/home health experience; a valid driver’s license is required.
