RN Case Manager
Days, Monday to Friday, from 7:30 AM to 4:00 PM. This position does not require on-call duties, holiday work, or weekend shifts.
The RN Case Manager will be responsible for assessing discharge needs, addressing social determinants of health, and facilitating discharge planning and referral processes for acute medical and surgical patients. The typical caseload will be around 20 patients per day.
Minimum of 3 years of experience in acute care case management.
Current certifications in Basic Life Support (BLS) or Basic Cardiac Life Support (BCLS) are required.
