- Minimum of 3 years of experience in a clinical setting
- Active registered nurse license in Tennessee or a Master’s in Social Work with an active unrestricted license
- Strong customer service skills and effective time management abilities
- Must reside within specific counties:
Maury, Marshall, or Giles - Proficient in basic Microsoft Office applications and email communication
- Experience working in a community health setting
- Familiarity with home and community‑based service authorizations
- Knowledge of the Nurse Licensure Compact Act
- Additional certifications in relevant healthcare fields
- Previous experience in a care coordination role
