Health Connect America, Inc. is seeking a Therapist to provide community-based and office-based behavioral health services to children, adolescents, adults, and families. In this role, you will conduct biopsychosocial assessments, develop individualized treatment plans, and deliver evidence-based individual, family, and group therapy in homes, schools, and community settings. You will collaborate closely with a multidisciplinary team, coordinate care with community partners and agencies, maintain thorough clinical documentation, and participate in supervision and ongoing training.

Responsibilities
  • Conduct comprehensive clinical and biopsychosocial assessments for children, adolescents, adults, and families.
  • Develop and implement individualized treatment plans using evidence-based therapeutic modalities.
  • Provide individual, family, and group therapy in homes, offices, schools, and community settings.
  • Deliver crisis intervention and safety planning as needed to support client stability and safety.
  • Collaborate with a multidisciplinary team and coordinate care with community partners, schools, and agencies.
  • Maintain accurate, timely clinical documentation in the EHR in accordance with regulatory and agency standards.
  • Participate in supervision, case consultation, and ongoing professional training and development.
  • Engage clients and families with a trauma-informed, culturally responsive, and client-centered approach.
  • Monitor treatment progress, adjust interventions, and support discharge and aftercare planning.
  • Support organizational goals, quality standards, and ethical practices aligned with agency values of compassion and integrity.
