  • Responsible for outreach contacts with homeless mentally ill individuals or homeless individuals that present with co-occurring disorders.
  • Completes P.A.T.H. Program Intakes with eligible consumers
  • Screens and provides psycho-social assessment of program participants
  • Provides advocacy for homeless consumers until discharged from the program
  • Develops linkage plan with goals, review and update of linkage plan as needed
  • Monitors consumer progress toward obtaining goals
  • Make referrals for housing, psychiatric evaluations, vocational services, Mobile Crisis Team, health care services, mental health center or mental health services or other services as deemed necessary
  • Provides assistance in obtaining income support
  • Conducts alcohol and drug screening for educational assessment and referral purposes
  • Maintains all documentation and keeps records current to include 90 Day P.A.T.H. Program Plan, Tenn Care eligibility, progress notes and case closures
  • Meets with P.A.T.H. Program Participants face to face by making shelter and treatment facility visits, as frequently as needed to foster stability and decline in program recidivism
  • Conducts life skills training, as needed
  • Provides assistance to link consumers to Tenn Care Partners
  • Participates in area homeless events and community partner and contract required meetings.

Education:

BA/BS in Social Work, Psychology, or other Behavioral Science disciplines. Knowledge of homeless outreach programs preferred and the ability to conduct alcohol and drug education groups.

Experience:

Experience with working with severely persistently mentally ill (SPMI) individuals or individuals with co-occurring disorders. Ability to work with minimal supervision, while demonstrating integrity and representing the agency within the community. Knowledge of community resources.
