Job Summary

Elysian Hospice is seeking a compassionate and experienced Admission RN to serve as the first clinical point of contact for patients entering hospice care. In this role, you will lead the admissions process by completing comprehensive assessments, determining hospice eligibility, and initiating the plan of care. You will work closely with patients, families, physicians, and the interdisciplinary team to ensure a smooth, timely transition into hospice services while delivering high-quality, patient-centered end-of-life care.

Qualifications
  • Graduate of an accredited Diploma, Associate, or Bachelor’s degree in Nursing
  • Current Registered Nurse (RN) license (Compact preferred)
  • Minimum 2 years RN clinical experience
  • At least 1 year of hospice experience (required) with in-home patient care
  • Strong knowledge of hospice regulations, Medicare guidelines, and end-of-life care practices
  • Experience with clinical assessments, admissions, and care planning
  • Excellent communication, assessment, and interpersonal skills
  • Ability to work independently in a field-based environment
  • Reliable transportation with valid driver’s license and auto insurance
  • Proficiency with EMR systems (Home Care Home Base preferred)
Essential Functions
  • Perform hospice admissions and initial patient assessments in the home setting
  • Evaluate patients using clinical judgment and LCD criteria to determine hospice eligibility
  • Develop and initiate individualized plans of care based on comprehensive assessments
  • Coordinate with the Interdisciplinary Group (IDG/IDT) to ensure patient and family needs are met
  • Communicate effectively with attending physicians, hospice physicians, and care teams
  • Provide patient and family education on disease progression, hospice philosophy, and care expectations
  • Ensure timely and accurate clinical documentation and submission of required paperwork
  • Apply infection control practices in accordance with OSHA and regulatory standards
  • Participate in QAPI initiatives and ongoing performance improvement efforts
  • Participate in a rotating on-call schedule, providing after-hours triage, support, and visits as needed
  • Maintain productivity standards while delivering high-quality, patient-centered care
