LPN Case Manager

The LPN is responsible for delivering direct nursing services to consumers living in agency homes in the Nashville and/or Murfreesboro Developmental Disabilities Services program. The role coordinates with other medical professionals as needed and provides medical oversight in accordance with Tennessee state guidelines.

The position requires the employee to be currently licensed as a Licensed Practical Nurse with the State of Tennessee and have one year of nursing experience. Experience working with individuals who have developmental disabilities and/or mental health concerns is desirable. Knowledge of the guidelines established by the state of Tennessee for community support programs for individuals with developmental disabilities is preferred.

• Monitor consumer safety related to medical care and medication administration.
• Conduct medication administration training as needed.
• Makes home visits when requested.
• Review clients’ medical status on a regular basis.
• Complete paperwork required for training and home visits.
• Interact with consumers in supportive, therapeutic ways and use effective techniques to reinforce behaviors.
• Aid in meeting the terms of the consumers’ Individual Support Plans (ISPs).
• Assist in ensuring medical files are current and accurate.
• Submit required paperwork in a timely manner.
• Attend training as needed.
• Assist in supervising LPN staff.
• Complete PSR’s on clients in a timely manner.
• Other job duties as assigned by Program Director.
