Position Summary

We are seeking a dedicated Licensed Practical Nurse (LPN) to provide skilled nursing services, monitor client health, and ensure high-quality care according to individualized plans. This role collaborates with RN Case Managers, caregivers, and interdisciplinary teams to promote health, safety, and independence.

Key Responsibilities
  • Perform skilled nursing assessments and implement care plans under RN direction.
  • Administer medications and treatments safely and accurately.
  • Complete clinical documentation and report changes in client condition promptly.
  • Communicate effectively with clients, families, and care teams to ensure continuity of care.
Qualifications
  • Graduate of an accredited LPN program.
  • Active LPN license in good standing in Tennessee.
  • Minimum 1 year of nursing experience preferred; home health experience a plus.
  • Strong clinical judgment, documentation skills, and ability to work independently.
