Northbrooke Post-Acute, a 120-bed Skilled Nursing Facility, located in Jackson, Tennessee, provides wound care, rehabilitation, and skilled services. The Certified Nursing Assistant role supports assigned residents with routine daily activities and nursing care services in accordance with the resident’s assessment and care plan, and as directed by supervisors.

• Use the wristband or photo to identify residents before administering treatments, serving meals, etc.
• Record all entries on flow sheets (ADLs, I&Os, percentage of meals eaten, vitals, bowel & bladder) in an informative, descriptive, and legible manner; record additional entries as requested by the charge nurse.
• Use only authorized abbreviations established by this facility when recording information.
• Report all changes in the resident’s condition to the Nurse Supervisor/Charge Nurse as soon as practical.
• Report all accidents and incidents observed as they occur.
• Transport residents to assigned locations for activity.
• Monitor and supervise residents at all times to ensure resident safety.
• Assist in passing meal trays at meal times; ensure the correct tray is given to the correct resident.
• Assist and participate in a hydration program.
• Assist patients at meal times.

• High school diploma or GED.
• Licensed Certified Nursing Assistant in Tennessee.
• Active BLS/CPR certification.
