The LPN is responsible for providing quality patient care and performing skilled care in the patient’s home.

Key responsibilities include:

• Provide services within the scope of practice as defined by state laws, in accordance with the plan of care and in coordination with other members of the health care team.
• Instruct the patient’s family in the care of the patient and maintain a healthy environment for the patient.
• Observe, record, and report to the supervising nurse and/or physician the patient’s response to treatment and changes in their condition daily.
• Collaborate closely with physicians and RNs, document care accurately and promptly, and may not alter the plan of care.

• One year of clinical LPN experience.
• Current LPN license in the state where services are rendered.
• Current CPR certification.
• Valid driver’s license, vehicle insurance, and access to a dependable vehicle or public transportation.
• Ability to reliably commute to multiple patient residences daily within the geographic area assigned.
