Position OverviewAs a Wound Care Nurse
, you would be responsible for overseeing the wound care program for the entire facility, ensuring best practices are followed, and coordinating care across interdisciplinary teams. The ideal candidate is experienced in wound care, has excellent assessment and communication skills, and is passionate about improving patient outcomes.
- Oversee the wound care program for the facility, ensuring compliance with federal, state, and facility standards.
- Conduct comprehensive skin and wound assessments; develop, implement, and monitor individualized treatment plans.
- Provide direct wound care as needed, while primarily functioning in a leadership and coordination role.
- Collaborate with physicians, nursing staff, therapy, and dietary teams to ensure optimal wound healing and prevention strategies.
- Track, document, and report wound healing progress, infection control issues, and trends in wound development.
- Educate and train nursing staff on wound care best practices, documentation, and prevention strategies.
- Participate in Quality Assurance and Performance Improvement (QAPI) activities as they relate to skin and wound care.
- Serve as the facility’s point person for wound-related audits, surveys, and external inspections.
- Active RN or LPN license in the state of Tennessee.
- Wound Care Certification (e.g., WCC, WOCN) preferred but not required.
- Minimum of 1 year in wound care strongly preferred.
- Skilled nursing facility experience preferred.
- Strong clinical assessment and critical thinking skills.
- Ability to lead, educate, and collaborate effectively with a multidisciplinary team.
- Excellent organizational, documentation, and communication abilities.
