Maplewood Health Care Center in Jackson, Tennessee is seeking a dedicated MDS Coordinator (LPN or RN) to join our care team in a full-time role. You'll lead the MDS/RAI process, ensure accurate assessments, and support care planning while collaborating with a multidisciplinary team.
Ideal candidates hold an active Tennessee LPN/RN license, have at least 3 years of nursing experience with supervisory responsibilities, and complete MDS training within six months. RAC certification is preferred.
