Summary of Position
Provides for the safety, permanency and well‑being of foster care clients, serving as the primary link between Centerstone, DCS, foster families, biological families, schools, and services needed. Conducts in‑home visits and manages issues and concerns of foster children/families, provides linkage and referral to other services and supports to enhance family functioning.
Essential Duties & Responsibilities- Learn and apply Centerstone and DCS policies and procedures.
- Attend and participate in CFTM’s.
- Link clients/families to community resources.
- Assist families with medication issues when necessary.
- Help achieve all EPSDT and dental requirements, including necessary follow‑up.
- Sign off on MAR in the foster home.
- Complete all TFACTS and Centerstone EMR requirements on time.
- Complete all required face‑to‑face visits per DCS policy.
- Perform documentation within 48 hours of contact.
- Develop a plan of care within 30 days of admission and every 90 days thereafter, complete with goals, objectives, interventions.
- Use behavior management and crisis management skills when needed.
- Coordinate case management services with school personnel.
- Coordinate, transport and supervise visitation as agreed upon in the CFTM.
- Assist foster parents with transportation when needed.
- Attend court as required.
- Coordinate community service projects when necessary.
- Complete required training and continuing education.
- Complete timesheets in a timely manner.
- Participate in all staff supervision activities.
- Participate in 24/7 on‑call coverage.
- Strong communication skills.
- Strong writing skills.
- Willingness to learn about and provide trauma‑informed care.
- Ability to work independently and as part of a team.
- Education Level: Bachelor’s degree required. Degree in Psychology, Social Work, Criminal Justice or other behavioral health care field preferred.

Full time – $16.50–$23.10 per hour.
