general description of position
the supportive living companion works under immediate supervision and provides individualised services and activities within the consumer's home and community. The role ensures services are delivered as designed to assist in acquiring, retaining, and improving the consumer's self-help, socialisation, and adaptive skills. It reports incidents to the appropriate contact, completes activity logs linked to person‑centered plan objectives, provides required documents, and maintains compliance with personnel documentation requirements.
schedule: monday–friday, 10:00 am–5:00 pm (gb/1223).
essential duties and responsibilities- provide services and activities to the consumer within their home and community as determined by goals and objectives in a person‑centered plan.
- train and support the individual in independent living skills, home management, cooking, cleaning, socialisation, supported employment, and other identified skills needed to foster independence.
- follow procedures within 24 hours for incidents identified according to department of human services policy.
- complete activity logs that occur during the work time frame as identified within the person‑centered plan objectives.
- report any change affecting the individual receiving services, as applicable.
- remind the consumer when it is necessary to take or refill medication.
- complete and provide all required documents.
- meet initial and ongoing annual training requirements.
- maintain compliance with required personnel documents.
- perform any other related duties as required or assigned.
high school diploma or ged; or one year of relevant, supervised work experience with public health, human services or other community service agency; or two years of verifiable successful experience working with individuals with developmental disabilities.
- the ability to understand written activity plans, execute instructions, and document services delivered.
- effective communication skills with consumers.
- access to emergency service systems.
- access to required transportation services.
- successfully passed a criminal background check, adult and child maltreatment registry checks. Background and adult maltreatment checks must be repeated every five (5) years, child maltreatment checks every two (2) years.
- completed a drug screen prior to employment in accordance with the organization’s policies and procedures.
must be able to pass applicable background checks and drug screen. Any offer of employment will be based on the results of background checks and drug screen.
salary: $12.66 – $13.16 per hour.
