- Must be a fully enrolled Florida Medicaid Provider.
- Credentialed with Denta Quest Dental Insurance.
- Credentialed with Liberty Dental Insurance.
- Providing a full range of general dental services including comprehensive dental exams, preventive services, restorations, removable complete and partial dentures and extractions within the scope of a general dentist.
- Experience provide a full range of general dental services.
- DDS or DMD degree.
- Active Florida Dental License.
