Physical Therapist – Orthopedic Outpatient Center

Location: Jacksonville/ St. Augustine area

Employment Type: Full‑Time, Part‑time

Department: Rehabilitation Services

Reports To: Clinic Director / Lead Physical Therapist

Position Summary

We are seeking a passionate and experienced Physical Therapist to join our orthopedic outpatient team. The ideal candidate thrives in a collaborative setting, demonstrates strong clinical reasoning, and is dedicated to helping patients achieve optimal functional outcomes.

Key Responsibilities
  • Evaluate and treat patients with orthopedic conditions using evidence‑based techniques.
  • Develop individualized treatment plans based on clinical assessments and patient goals.
  • Collaborate with physicians, surgeons, and other healthcare professionals to coordinate care.
  • Document patient progress accurately and timely in EMR systems.
  • Educate patients on injury prevention, post‑operative care, and home exercise programs.
  • Participate in team meetings and contribute to clinical improvement initiatives.
  • Mentor PT students or new staff as needed.
Qualifications
  • Doctor of Physical Therapy (DPT) or equivalent from an accredited program.
  • Current state licensure as a Physical Therapist.
  • Strong manual therapy and therapeutic exercise skills.
  • Excellent communication and interpersonal abilities.
  • Ability to manage a full caseload in a fast‑paced environment.
