Responsibilities Total Rewards of a Allied Health Professional The position is responsible for program oversight, monitoring and administration, serving as the primary contact with local Public Housing Authorities (PHAs) and providing case management-based services (directly or through referral) necessary to ensure that Veteran care is coordinated with the VA Medical Center, community health or mental health providers, and agencies that serve homeless populations.
Veterans eligible for HUD/VASH come from a wide geographic area. Their health and mental health needs may be complex and require a high degree of clinical oversight and creative problem solving. Services are provided in healthcare settings and in the community. The HUD/VASH Program requires continuous coordination with the community's homeless providers and coalitions as well as with other local, state, and federal agencies.
Major duties of the HUD VASH Senior Social Worker can include:
Works closely with the VA medical center's homeless programs, residential and community care programs, and domiciliary programs to ensure that coordination between these entities is facilitated and veterans direct care, referral, and follow up is contiguous and based on a continuum of services. Independently provides clinical psychosocial and case management services at an advanced practice level to eligible veterans and their family members in the VA Supporting Housing (VASH) Program to meet biological, psychological, social and environmental needs.
Provides support for the Health Care for Homeless Veterans (HCHV)/Community Care Programs including outreach activities, task forces and special assignments. Provides support to the local Medical Center through the assignment of special projects and/or committees. Provides support and mentoring to social work interns and newly hired social workers. Travel to patient homes and outreach sites is required via government vehicle so a valid driver's license is required for this position.
This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.
