Assistant Director of Nursing​/MDS Coordinator

The Pavilion at Jacksonville is seeking an Assistant Director of Nursing / MDS Coordinator to support clinical leadership and oversee the MDS process within the facility.

This dual-role position works closely with the Director of Nursing to ensure regulatory compliance, accurate clinical documentation, and effective care planning. The ideal candidate is organized, detail-oriented, and experienced in both clinical leadership and the MDS process within a skilled nursing environment.

Key Responsibilities

• Support the Director of Nursing in overseeing nursing operations

• Coordinate and complete MDS assessments in accordance with regulatory requirements

• Ensure accuracy and timeliness of MDS submissions

• Participate in care plan development and interdisciplinary meetings

• Monitor clinical documentation to support reimbursement and compliance

• Assist with survey readiness and regulatory compliance

• Help supervise nursing staff and support clinical leadership

Qualifications

• Active RN license required

• Prior Skilled Nursing Facility experience required

• MDS experience required

• Strong understanding of Medicare, Medicaid, and reimbursement processes

• Knowledge of care planning and regulatory compliance

• Strong organizational and communication skills
