Family Advocate Liaison

Nemours is seeking a Family Advocate Liaison to join our team in Jacksonville, FL.

The Family Advocate Liaison will be supporting communication and engagement between families of children with complex health care needs and the healthcare system. The Family Advocate Liaison serves as a liaison to promote information sharing, gather family input, and support family-centered care. The role collaborates with interdisciplinary teams and community partners to support program activities, quality improvement initiatives, and outreach efforts.

The Advocate also assists with coordinating activities, collecting feedback, and contributing to improvements in service delivery.

Essential Function:

  • Maintain regular communication with families to provide support, share information, and encourage engagement in care and program activities within the assigned program area.
  • Prepare documentation of non-clinical patient and family interactions, outreach activities, and referrals in the electronic medical record in accordance with departmental guidelines.
  • Serve as a liaison between families, healthcare providers, and community partners to support coordination of services and effective communication.
  • Gather and communicate feedback from families and staff to identify needs, concerns, and opportunities to improve services.
  • Assist with planning, coordination, and participation in meetings, support groups, and family engagement activities.
  • Support quality improvement efforts by contributing to data collection, participation in PDSA cycles, and ongoing program initiatives in accordance with departmental goals.
  • Participate in community outreach and advocacy activities to promote family-centered care and increase awareness of available resources.
  • Assist with the development, implementation, and evaluation of program activities and initiatives.
  • Establish and maintain effective working relationships with families, staff, and community partners.
  • Requirement:

    High school Diploma or equivalent

    6 months or more of job related experience
