The
Catholic Charities Jacksonville Regional Office
is seeking a full-time, non-exempt Emergency Financial Assistance Outreach Coordinator. This is a in-office 40 hour a week position, Monday to Friday, 8 am to 4:30 pm, in-office position is located in Jacksonville, FL. The salary range is $21 to $22 an hour, commensurate with experience and qualifications.

Job Summary

Responsible for coordinating outreach, community engagement, and referral development for the Emergency Financial Assistance Program funded through the Housing Opportunities for Persons with AIDS (HOPWA) contract. Builds and maintains relationships with healthcare providers, community organizations, and other referral partners to increase program awareness and connect eligible individuals with available services. Conducts proactive outreach to identify individuals and families affected by HIV/AIDS who are experiencing housing instability or financial crisis and facilitates timely access to housing and financial assistance resources.

Essential Duties and Responsibilities
  • Develop and maintain collaborative relationships with hospitals, healthcare providers, health departments, community-based organizations, and other referral partners to promote program awareness and facilitate referrals.
  • Conduct regular outreach visits, presentations, and educational activities with community partners regarding program eligibility, available services, and referral procedures.
  • Represent Catholic Charities at community meetings, coalitions, networking events, and other collaborative activities to strengthen partnerships and expand referral networks.
  • Identify and engage individuals and families affected by HIV/AIDS who may be experiencing housing instability, financial crisis, or other needs that may qualify them for program assistance.
  • Coordinate with Intake, Case Management, and other program staff to ensure timely follow-up on referrals, appropriate connections to services, and continuity of care.
  • Assist in developing and implementing outreach strategies, materials, and activities designed to increase program visibility, awareness, and access among eligible populations.
  • Track outreach activities, community partnerships, referral sources, and outcomes to support program evaluation, performance monitoring, and required reporting.
  • Maintain accurate, complete, and timely documentation in accordance with agency, program, contractual, and funder requirements.
  • Support overall program performance and compliance with HOPWA contractual, programmatic, documentation, and reporting requirements.
  • Other duties as assigned.

  • Treat clients, coworkers, volunteers, and community partners with dignity and respect, demonstrating sensitivity to the cultural, socioeconomic, and spiritual characteristics of the populations served.
  • Maintain the confidentiality and security of client, employee, and agency information in accordance with agency policies and applicable laws and regulations.
  • Maintain regular and effective communication with the supervisor regarding outreach activities, referral trends, community partnerships, emerging needs, challenges, and other matters requiring guidance or attention.
  • Attend required agency, departmental, and program meetings and participate in training, in-service activities, continuing education, conferences, and workshops necessary for professional development and compliance.
  • Complete and maintain all documentation, records, and required reports in a timely, complete, and accurate manner and in accordance with applicable regulatory, statutory, contractual, grant, and agency requirements.
  • Comply with all applicable Diocese of St. Augustine and Catholic Charities Bureau, Inc. policies, procedures, standards, and requirements.
  • Perform other duties as assigned to support program and agency operations.
Knowledge, Skills, and Abilities
  • Strong knowledge of community resources and healthcare systems, with excellent interpersonal and relationship-building skills to develop and maintain effective partnerships with healthcare providers, community organizations, referral sources, and other stakeholders.
