SUMMARY
The Family Nurse Practitioner provides direct pediatric and adult care to the patients of the Duval Family Health Center. This position is part time and on an as needed basis. The Family Nurse Practitioner is fully licensed in the State of Florida. He/she actively collaborates with other healthcare providers to ensure holistic patient care, and maintains documentation and patient records in accordance with regulatory and Center policies.

ESSENTIAL DUTIES AND RESPONSIBILITIES
1. Maintains productivity levels while providing quality medical care to eligible child and adult patients including diagnosis, preventative care and treatment, medication management and referrals for other services as needed
2. Promotes positive health behaviors and self-care skills through education and counseling
3. Evaluates effectiveness of treatment programs on patient progress in resolving identified problems and moving towards defined objectives
4. Completes all relevant charting, documentation and follow-up. Maintains patient-related paperwork, including federal and state mandated forms, patient diagnostic records and progress notes
5. Maintains knowledge of new developments in family care by reading professional literature, attending courses and seminars and establishing/maintaining contact with other healthcare agencies
6. Participates in quality assurance/improvement and risk management efforts
7. Coordinate patient care with members of the Housing Team and other Clinic departments with a particular focus on integrated care.
9. Participate in all appropriate team meetings and required training sessions.
10. Maintain active, unrestricted APRN license.
11. All other duties as assigned.

The Family Nurse Practitioner is sensitive to cultural, spiritual and practical needs of resident, families, guests and staff members in all interactions. He/she provides positive feedback and comments about the quality of work and effort that have been offered by patients, residents, families and coworkers, and adjusts priorities and schedules to meet specific customer needs. The Family Nurse Practitioner reports and/or corrects problems related to safety, cleanliness and comfort of resident, patient and work areas, and follows Universal Precautions and Infection Control techniques.
He/she demonstrates an understanding of their role in facility safety plans and procedures related to fire, weather and other disasters.
EDUCATION and/or EXPERIENCE
Qualifications
- Florida licensed APRN with current valid, unrestricted Florida medical license
- Board certification
- Minimum of 2 years experience in a family care clinical setting
- Excellent interpersonal and time management skills
- Ability to pass a Level II Background Screening
