We are looking to add a Licensed Practical Nurse at Baptist Primary Care Ponte Vedra. This is a full‑time, day‑shift opportunity Monday - Friday from 8:00 AM until 5:00 PM. This large primary care office is conveniently located in Ponte Vedra, close to J Turner Butler Boulevard. The primary responsibility of this LPN position is to assist with phone triage and contacting patients.

As the Licensed Practical Nurse, you will provide nursing assistance to physicians and maintain medical documentation. This LPN supports the clinical and call‑center operations of a high‑volume family medicine and pediatric primary care practice. The role includes utilizing approved pediatric telephone triage protocols, providing high‑quality in‑office clinical support, and maintaining adherence to the CDC/ACIP pediatric immunization schedule. The nurse also provides high‑quality call‑center support by assessing symptoms, determining appropriate levels of care, documenting encounters, and assisting patients and families with clinical guidance.
