The Lead Advanced Practice Provider (APP) for the Division of Nephrology & Hypertension serves as both a clinician and operational leader for NP/PAs who engage in practice and education. The APP team members serve patient care needs in the inpatient and outpatient setting which includes acute and chronic dialysis therapies. This role integrates advanced clinical practice with team leadership to ensure high-quality, coordinated, and efficient nephrology care.
This position provides direct patient care across the continuum of nephrology services while overseeing the performance, development, scheduling, and operational effectiveness of the NP/PA workforce.
Lead APP responsibilities
- Serve as liaison between NP/PA staff, physician leadership, and administration.
- Ensure appropriate staffing and NP/PA coverage for hospital days, nights, weekends, holidays, and in-house overnight shifts.
- Provide coaching, evaluations, and performance feedback.
- Lead staff meetings and communicate workflow updates.
- Support orientation and onboarding for new employees.
- Identify and resolve operational issues across the service.
- Contribute to policy creation, process improvement, and operational planning.
Clinical responsibilities
- Work independently to the full scope of practice in compliance with state licensure.
- Perform and document in the medical record detailed patient history and physical examinations.
- Coordinates daily rounds with the attending physician teams; presenting patient report to develop, collaborate and implement individualized plan of care.
- A minimum of 20% in-hospital coverage, which may include days, nights, or weekends
- Occasional outpatient clinic visits, including rounding in outpatient dialysis unit.
- Follows assigned patient population throughout the shift.
- Deliver patient education on the spectrum of CKD care and renal replacement therapies, including dialysis modality options.
- Coordinate care of ESRD patients with outpatient dialysis centers from admission to discharge.
- Prescribe appropriate pharmacological treatments and therapeutic procedures, including dialysis and continuous renal replacement therapies, as per accepted evidence-based guidelines and expected institutional protocols.
- Monitor effectiveness of therapeutic interventions.
- Participate in the delivery of Tele-Health for Advanced Care at Home Patients or other video visits for ambulatory care.
- Make appropriate referrals as needed.
- Examine plan of care for cost-effectiveness and achievement of expected outcomes.
- Assist with ongoing division/departmental process improvement and policy development.
- Actively participate in educational lectures as part of the MD and APP fellowship program, grand rounds, quality improvement and faculty meetings.
- Precept, mentor and instruct students, graduates, and novice NP/PAs.
