The Department of Hospital Internal Medicine seeks a qualified Nurse Practitioner or Physician Assistant to join our Advanced Care at Home (Hospital at Home) division. The position offers a unique opportunity to provide a mix of in‑person and virtual patient care to hospitalized patients 24/7 via our virtual platform. You will work with physicians, nurses, case management, pharmacy, and administration to provide quality and strategic care to patients in the comfort of their homes using community service providers.

As an NP/PA hospitalist within the Advanced Care at Home program, you will practice at the top of your scope of practice / licensure, manage a medically complex and diverse patient population, and participate in daily rounding, coordination with consultants, patient education, and discharge planning. Providers are expected to stay current with evidence‑based medicine and maintain 24/7 availability, including nights, holidays, and weekends on a rotation structure.

Due to the complexity of our patient population, a minimum of two to three years of inpatient clinical experience is preferred in either acute care medicine (hospital, ER) or clinic practice as a certified NP/PA. Hospital Internal Medicine experience preferred. New graduates will not be considered at this time.

Nurse Practitioner: Masters‑prepared graduate of an accredited nursing program and NP program. Current FL RN and APRN license and certification as an FNP, ANP or ACNP.

Physician Assistant: Graduate of an accredited PA program with current PA certification by NCCPA and registration as a PA by the FL State Board of Medical Practice.

• Excellent written and oral communication skills.
• Flexibility to adjust schedule to meet patient and staff needs.
• Ability to manage time and stress effectively and adapt to unpredictable situations within the patient care setting.
• Strong computer skills in an Electronic Medical Record preferred.

Nurse Practitioner or Physician Assistant license to practice in the State of Florida. BLS is required (American Heart Association or Red Cross).

Full Time – Core APP hours 7a‑7p or 9a‑9p Monday through Friday pending shift assignment. Providers participate in 24/7 care, including nights, holidays, and weekends on a rotation structure.

Education, experience and tenure may be considered along with internal equity when job offers are extended.
