Pace Place provides coordinated medical and social services to elders who want to remain in their home but need support to do so. Pace Place provides Medical, Rehabilitation, In-Home Assistance, Transportation, Social Work, and Day Center services for patients in our care.

Position Summary

The PACE RN Care Manager is responsible for delivering high-quality nursing care across various settings including the clinic, home, and short-term care facilities. This role includes administering treatments, assessing participants’ health, coordinating care with the Interdisciplinary Team (IDT), educating participants and families, and ensuring compliance with all regulatory standards. The RN Care Manager plays a critical role in triaging participant needs, maintaining clinical records, and providing holistic care for a frail or elderly population.

$2500 Sign on Bonus offered

Shift: M-F with on call rotation

Essential Functions
  • Deliver clinical nursing care in accordance with established professional standards.
  • Conduct in-person comprehensive assessments for new enrollees and ongoing participants.
  • Monitor and record vital signs; recognize abnormalities and take appropriate action.
  • Perform clinical tasks including wound care, injections, TB tests, pill counts, and lab specimen collection.
  • Administer and reconcile medications per physician orders and organizational policy.
  • Participate in the IDT and Intake and Assessment meetings as required.
  • Provide participant education and counseling on health maintenance, treatments, and medications.
  • Maintain timely and accurate documentation of nursing notes and participant records.
  • Respond to medical emergencies and provide appropriate interventions and transport coordination.
  • Ensure consistent communication with team members regarding participant care.
  • Comply with all relevant state and federal regulations regarding care coordination, assessments, grievances, and documentation.
  • Participate in on-call nursing rotation for after‑hours care needs.
  • Follow infection control and safety procedures including handling of biohazardous materials.
  • Support and participate in quality improvement initiatives.
Qualifications Knowledge and Critical Skills
  • Strong clinical nursing skills with an emphasis on geriatric care
  • Ability to triage and manage participant care effectively
  • Excellent communication and interpersonal skills
  • Ability to collaborate within an interdisciplinary team
  • Familiarity with regulatory and compliance requirements
  • Skilled in documentation and use of clinical records systems
  • Knowledge of growth, development, and age‑specific care for adult and geriatric populations
  • Only act within the scope of his or her authority to practice
  • Understanding of HIPAA and confidentiality standards
Experience
  • Minimum of 1 year experience working with a frail or elderly population; if less, must undergo specialized training upon hire
  • Preferred: 2 years of nursing experience in hospital, nursing home, or community-based setting
  • Must meet all PACE organization competencies before working independently
License / Certification
  • Current Registered Nurse (RN) license
  • BLS Certification
Education
  • Associate Degree in Nursing required
  • Bachelor’s Degree in Nursing preferred
Other Requirements
  • Medical clearance for communicable diseases and current immunizations required prior to participant contact
