Licensed Practical Nurse (LPN) delivering in‑home nursing care to pediatric patients in St. Augustine, FL.

Schedule:

Days, 7 a.m. – 7 p.m. Care setting:
Feeding Tube; nurse must go to school with client.

• Provide comprehensive nursing services in accordance with Family First policies, state and federal regulations, and the Nursing Practice Act.
• Maintain an updated clinical record on each patient at all times, meeting required deadlines for documentation.
• Evaluate the effectiveness of nursing services to the patient and family on an ongoing basis.
• Consult with physicians regarding alterations of patient care plans, document changes, and report all changes to the nurse supervisor.
• Support Family First’s mission by mentoring and/or coaching new employees during patient‑specific orientations.
• Educate patients and their caregivers on topics related to patients’ care needs.
• Participate in the Quality Assurance and Performance Improvement program (QAPI) as requested.
• Continuously strive to improve nursing care skills.
• Attend and/or complete all mandatory training and in‑service sessions as assigned.
• Submit required personnel file documentation in a timely manner.
• Adhere to Family First compliance and ethics expectations.
• Maintain patient and family confidentiality.
• Maintain professional, supportive, and responsive interpersonal communication skills.
• Perform additional duties as assigned.

• LPN/LVN nursing degree and unencumbered LPN/LVN license in the state of employment or a Multi‑State license through the Enhanced Nurse Licensure Compact.
• Minimum of 1 year of demonstrated experience as an LPN/LVN.
• Valid driver’s license with proof of insurance in applicable states.
• Clear background screening.
• Working knowledge of Nurse Practice Acts.
• Valid and current CPR card/certificate with an in‑person instructor‑led, hands‑on skills check.
• Clear Hepatitis
  
  B screening in applicable states.
• Clear tuberculosis (TB) screening in applicable states.

• Work performed in patients’ homes; may not be substantially exposed to adverse environmental conditions.
• Training may occur in an office setting.
• Must be able to lift up to 35 pounds without an assisted lifting device.
• Frequent use of office machinery, including telephone, computer, and printer.
• Travel to patients’ homes for care delivery.
• Shift lengths vary (8, 10, or 12 hours) and may require availability any day of the week, Sunday through Saturday.
