The Licensed Practical Nurse (LPN) at WE LEVEL UP is responsible for providing high-quality nursing care to patients under the supervision of registered nurses and healthcare practitioners. The LPN will ensure the comfort, safety, and well-being of patients while adhering to established protocols and procedures. This role is vital in supporting the healthcare team and contributing to the overall patient care experience.
Position Responsibilities:
Administer medications and perform treatments as prescribed by physicians and registered nurses.
Monitor patients' vital signs and report any changes or concerns to the healthcare team.
Assist in developing and implementing patient care plans.
Provide direct patient care, including bathing, dressing, and feeding patients as needed.
Maintain accurate and detailed patient records, documenting care provided and any changes in condition.
Collaborate with other healthcare professionals to ensure cohesive and comprehensive patient care.
Educate patients and their families on health maintenance and disease prevention.
Ensure compliance with all healthcare regulations and standards of practice.
Participate in team meetings and contribute to the continuous improvement of care processes.
Maintain a clean and safe environment for patients and staff.
Required
Education:
Completion of an accredited Licensed Practical Nurse (LPN) program.
Current and valid LPN license in the state of practice.
CPR Certification
Required Experience:
Minimum of 1-2 years of experience working as a Licensed Practical Nurse in a healthcare setting.
Experience in patient care, including monitoring vital signs, administering medications, and assisting with daily living activities.

Skills and Abilities:
Strong clinical skills with the ability to perform nursing procedures and provide patient care under the supervision of a registered nurse or physician.
Excellent communication skills to effectively interact with patients, families, and healthcare team members.
Ability to maintain accurate patient records and documentation.
Proficiency in using healthcare software and electronic medical records (EMR) systems.
Demonstrated ability to work effectively in a fast-paced environment and handle multiple tasks simultaneously.
Compassionate and empathetic approach to patient care.
Strong organizational skills and attention to detail.
Ability to adhere to all healthcare regulations and standards.
