Position: LPN or RN 1:1 Private Duty Pediatric Homecare - St. Augustine, FL (Nights) & (Weekend Days)

Key Responsibilities
  • Accountable for providing care based on licensure and certification within the scope of practice.
  • Maintains an updated clinical record on each patient at all times, meeting required documentation deadlines.
  • Evaluates the effectiveness of nursing services to the patient and family on an ongoing basis.
  • Consults with physicians concerning alterations of patient care plans and reports all changes to the nurse supervisor.
  • Supports the organization’s mission by mentoring and coaching new employees during patient‑specific orientations.
  • Educates patients and their caregivers on topics related to patients’ care needs.
  • Participates in the Quality Assurance, Performance Improvement (QAPI) program as requested.
  • Continually strives to improve nursing care skills.
  • Attends and completes all mandatory training and in‑services as assigned.
  • Submits required personnel file documentation in a timely manner.
  • Adheres to compliance and ethics expectations.
  • Maintains patient and family confidentiality.
  • Maintains professional, supportive, and responsive interpersonal communication skills.
  • Performs other duties as assigned.
Qualifications
  • LPN/LVN nursing degree.
  • Unencumbered LPN/LVN license in the state of employment (or multi‑state license through the Enhanced Nurse Licensure Compact).
  • One year of demonstrated experience.
  • Valid driver license with proof of insurance in applicable states.
  • Clear background screening.
  • Working knowledge of Nurse Practice Acts.
  • Valid CPR card/certificate with in‑person instructor‑led hands‑on skills check.
  • Clear HepB screening in applicable states.
  • Clear TB screening in applicable states.
  • Preferred:
    Multi‑state license, additional background screening, and state‑specific qualifications.
State‑Specific Requirements
  • Florida: Driver license not required; driver license screening and TB screening not prerequisites; completed during onboarding.
  • Illinois: No driver license required; staff are prohibited from driving patients.
  • Pennsylvania: Child Abuse and FBI background check required; TB screening with 2‑step PPD (or 1‑step with proof) required.
  • Texas: No additional requirements.

Work

Conditions
  • The work is performed in patients’ homes and may expose the employee to varying environments.
  • Training may also be conducted in an office setting.
  • Employee must be able to lift up to 35 pounds without an assisted lifting device.
  • Frequent use of office machines such as telephone, computer, and printer.
Schedule

The role requires travel to patients’ homes. Work schedule may include varying shift lengths (8, 10, or 12 hours) and may require availability any day of the week.
