We are seeking a compassionate and skilled LPN Licensed Practical Nurse to join our team at Fountains Rehabilitation at Mill Cove, a Senior Living Center nestled in the heart of Jacksonville, Florida. As a valued member of our care team, you will play a vital role in delivering exceptional healthcare services to our residents.

About the LPN Licensed Practical Nurse Role:

  • Provide high-quality patient care
    :
    As an LPN, you will be responsible for assessing, planning, implementing, and evaluating patient care plans in collaboration with our interdisciplinary team.

  • Collaborate with healthcare professionals
    :
    Work closely with our physicians, therapists, and other healthcare professionals to ensure comprehensive care for our residents.

  • Communicate effectively
    :
    Develop strong relationships with residents, families, and caregivers by communicating effectively and empathetically.

  • Maintain accurate records
    :
    Accurately document patient records, including medical histories, treatment plans, and progress reports.

LPN Licensed Practical Nurse Responsibilities:

  • Administer medications and treatments as prescribed by physicians
  • Monitor and report changes in patient condition to physicians and other healthcare professionals
  • Perform various diagnostic tests, such as vital signs and blood pressure checks
  • Assist with wound care, dressing changes, and other medical procedures
  • Provide emotional support and education to residents and families
