Ennoble Care is a mobile primary care, palliative care, and hospice service provider with patients across multiple states. Clinicians visit patients in their homes, delivering continuum of care for those with chronic conditions and limited mobility. Services include remote patient monitoring, behavioral health management, and chronic care management to ensure high-quality, patient-centered care.

Purpose

We are seeking a full-time Nurse Practitioner or Physician Assistant to provide primary care to patients in the Jacksonville, FL region. You will apply medical techniques and principles under physician supervision and care for a diverse patient population with chronic diseases.

Responsibilities
  • Provide high-quality primary and palliative care to patients in and around Jacksonville, FL
  • Assess health through physical examinations, collect and review medical histories, and develop care plans
  • Communicate effectively with office staff and clinical operations across internal platforms
  • Deliver patient-centered care to a defined panel of patients
  • Attend quarterly medical staff meetings
  • Develop impressions, diagnoses, and treatment plans
  • Build trust with patients, families, and caregivers during visits
  • Order diagnostic testing as medically necessary
  • Prescribe medications, supplies, and appropriate medically necessary interventions
Qualifications
  • Board Certified with a valid NP or PA license in the State of Florida
  • Strong clinical skills to accurately assess, diagnose, and treat patients
  • Positive demeanor and strong interpersonal skills
  • Ability to problem-solve and operate independently
  • 1–2 years of primary care experience with geriatric patients (hospice experience preferred but not required)
Compensation and Benefits
  • Salary:
    Full-time $105,000 – $130,000+ (based on experience and incentives); providers are paid per visit
